Sports Physicals

***Your student athlete will need a sports physical before he/she can participate in sports in the 2017-2018 school year. Please call your family health care provider for an appointment Now.

**Also, if your child has allergies (to any food or medication), or has asthma, your doctor needs to sign an Action Plan form. Please pick this form and the “Permission to Participate” form up at the school office.

Attention Parents of (2017-2018) 7th Graders:

Nebraska law (LB79-444) states that any student entering Seventh Grade for the first time is required to have a physical examination given by a physician, physician’s assistant or accredited nurse practitioner within 6 months prior to the entrance of the student in a Nebraska school. If the physical will also be an athletic physical, May is the earliest it can be done. The completed physical form is to be presented to the school no later than the first day of school in August. The medical clinics have the physical forms. There are also immunizations that your child is required to obtain and if your child has allergies (to any food or medications), or has asthma, your doctor needs to sign an Action plan form. Please pick up this form at the school office.
